Delayed loading implantology is a traditional technique that consists of placing prosthetic crowns after a certain time by inserting the dental implant. Delayed loading implantology is mostly chosen as the right option when the patient’s bone takes a long time (longer than normal) to complete osseointegration. It is a must to wait a few times before mounting the prostheses when the inserted implant is not yet solid and stable and cannot withstand the pressure of chewing. The waiting period varies from three to four months.
After the waiting period is over, a sufficient amount of bone is created around the implant, which is now stable. When thinking of delayed loading implantology, compared to immediate loading implantology, do not think there exists a difference in the aesthetic appearance or the functionality of the prostheses because there is none- the only difference is the healing time.
In simple and more understandable words: delayed loading implantology consists of three easy steps:
The dentist removes your old tooth.
Which is the healing time, and can last from three to four months. During this time the patient is allowed to wear dentures if preferred.
After the area completely heals, the dentist will then place the fixture and insert the prostheses.
Delayed loading implants work just like immediate loading implants, with the only difference of a gap between introducing the implant and fitting the replacement tooth into it.
What your dentist first does is cut your gum to give access to your jawbone. Next, the implant, which serves as a “channel” for the new replacement tooth, is fitted into the jaw. Once the implant is fixed, the dentist stitches up surgically your gum.
Differently from immediate loading implantation, in delayed loading implantation, your implant is given a few months to heal and completely osseointegrate with the jawbone. This, of course, means that the implant will fully be anchored into the bone, allowing the new replacement to be inserted into a solid foundation.
The waiting time between the implantation and the tooth replacement can vary from three to four months, all depending on the bones, the success of the implant, and the dentist’s opinions.
After the waiting period is over, the dentist will reopen the gum so that the tooth or teeth replacement can be inserted into the implant.
There exist several advantages when it comes to delayed loading implants, which are listed below:
Even though most patients prefer a short time of waiting and love spending less time at the dentist, the healing time is the most convincing reason when talking about delayed loading implants. That’s because the more time the jaw and implant are given to osseointegrate, the more likely the procedure to be a complete success.
We know what you are thinking: “rejection and failure” sound like a disadvantage, but hear us out. We are talking about rejection and failure compared to immediate loading implants. In delayed loading implantology is less likely to happen than in immediate loading implantology. If the body rejects the implant, the fixture will not osseointegrate, which means that it will not fix successfully into the jawbone. This happening can lead to discomfort, pain, and complete failure of the dental implant.
Though this might be more of an advantage for the dentists, seeing as they will feel more comfortable during the surgery and more confident of the surgery’s success, it is an advantage for you too- because, again, it means a higher chance of the surgery being successful.
This doesn’t need much explanation. Surgeries/ dental implants are easier to be performed meaning fewer expenses, which indirectly affects the price you will have to pay for them.
The biggest disadvantage delayed loading implantology has, is found in its name, “delayed”. As mentioned, most people don’t like going to the clinic often or waiting to get their teeth done, so maybe delayed loading implantology isn’t the right choice, seeing as already noted: delayed loading implantology means a waiting period of three to four months.
That said, the benefits of delayed loading implantology outweigh the disadvantage. There is a higher chance of success and you can save yourself the discomfort and pain, of your implant rejection.
Delayed loading implants and you become a good candidate for this method in the following clinical situations:
If there is a tooth infection, it is best to extract it and allow the area to heal from two to three months before placing an implant, that’s why immediate placement is avoided in such situations, and delayed loading implants are preferred.
When tooth extraction is followed by bone loss, be it from an infection or bone removal during the extraction, it is best to delay the implant placement. Implant success depends entirely on good implant stability and bone density.
Dental implants should always be placed using the delayed loading implantology methods for mola and som premolar teeth. Why? Because the root anatomy and size of molars and premolars make it difficult to achieve proper implant support and bone coverage.
Not precisely. If uncomfortable you can always choose to fill the gap by using dentures for the months the implants take to heal and osseointegrate. Of course, it is not the best solution, because oftentimes a patient undergoing surgery does it for the precise reason of escaping wearing dentures, but the long-term benefits of delayed loading implantology benefits are much stronger compared to the disadvantage of having to wear a denture for three to four months.
As acknowledged several times, it takes the implants from three to four months to fully integrate into the jawbone and become solid.
There are several reasons dental implants can fail and the signs of it are quite easily recognized and noted. Below you will find some of the signs:
If you have any of the above signs, please do not hesitate to contact us to schedule an appointment for a consultation or checkup.
People who have been users of dentures, bridges, or weak teeth for a long while, know just how important it is for the bite to be firm because the bite strength of an implant is from 80 to 90 percent of the strength of a real tooth. Unfortunately, this means that the strength of dental implants is not any bigger than the real teeth. Nevertheless, the strength is about the same and the care for your oral hygiene should be the same in both cases.
The delayed placement of the implants gives the area the right amount of time to be healed. Also, the osseointegration has the right time to happen completely and become more solid.
Recovery is different for everyone, however, you may expect the following:
Most patients experience elevated pain for three to four days, which can be easily managed with painkillers such as paracetamol or ibuprofen. After seven to ten days, the pain disappears and the medications can be stopped.
Any swelling related to the surgery lasts from 36 to 48 hours after the procedure, and it usually tapers over the next five to seven days. A good way of reducing the swelling in the first 24 hours is the use of ice.
As with any other procedure that happens in the mouth, delayed loading implants have a dietary restriction guide to be followed, which includes: water, juices, soup, puree, and soft food. You can return to normal food after seven days.
On the day of the surgery, it is advised to get plenty of rest. Exhausting activities are best to be avoided for the first two to three days. After three days, when you feel more comfortable you can walk or do very low-impact exercise. You can go back to normal activities one to two days after the surgery.
